Middle School Music Grant - Request Form

Grant deadline: Anytime between September 1st and January 31st during the eligible school year.

By submitting this Middle School Music Grant Request Form, the music teacher (“Grantee”) consents to the Eligibility Requirements, Guidelines and Conditions of the Grant.

Grantees/schools who received a Middle School Music Grant in the previous school year that did not/have not provided the required “Grant Performance Report” at the conclusion of the school year are ineligible to apply for a 2022/23 Grant or any future Grant until the Report has been provided.
