Grant Submission period: July 1 – January 31 of the eligible school year
Note: If your school or district requires the pre-approval of a Grant, DO NOT submit this Form until district approval is obtained.
By submitting this Middle School Music Grant Request Form, the music teacher ("Grantee") consents to the Eligibility Requirements, Guidelines and Conditions of the Grant.
Grantees/schools who received a Middle School Music Grant in a previous school year that did not fulfill the Grant Conditions are ineligible to apply for any future Grant until the Conditions have been met.
