Community Silent Servant Award - Nomination Form

A nomination can be submitted by a Nominator at any time of their choosing.

First, carefully review the Award Guidelines set forth on the Community Silent Servant Award main page. Nominations that do not meet the guidelines will not be accepted.

This form is used to nominate a single person for the Community Silent Servant Award only.

There are two available options to submit a nomination:

1. Preferred method:  Complete and submit the form electronically.
2. Hard Copy: Contact the Program Coordinator at info@prmfoundation.org to request a “hard copy” of the form.

Once this nomination has been received by the Foundation, you may be contacted to provide additional information.

When a nomination appears to meet all the guidelines set forth on the main Community Silent Servant Award page, it will be submitted to the Board of Directors for review.  The Nominator will be notified of the Board’s decision.

Note: ​Charitable organizations may submit one nomination per year.

  • Include “who, what, where, when, why”. Additionally,
    • How long they have been serving and frequency of their service.
    • The value of their service to others.

There are many wonderful people serving today to the benefit of society.  Several of these servants are worthy of being acknowledged, recognized and awarded.  We are hopeful that you will encourage other individuals/organizations to become a Nominator as well.
