Community Award Nomination​


​Nomination Instructions:

The award is ongoing, therefore there are no deadlines to submit a nomination.
Please review the guidelines, listed on the main Silent Servant Award page, to verify the nominee qualifies for the award.

To nominate an individual for the Community Silent Servant Award, you may either:
  1. Submit the electronic form located below (preferred method) or
  2. Contact the program Coordinator at info@prmfoundation.org to receive a hard copy of the form.

Once a nomination has been received, you may be contacted to provide additional information.  All nominations that meet the award criteria will be submitted to the PRMF Board of Directors for vote.  Please allow 2-4 weeks for this process.

Note: ​If you represent a charitable organization which has several volunteers worthy of the SSA, it is best to choose the most deserving as your single nominee.
